Our services are available to individuals over the age of 18 years with chronic and complex health conditions residing within the Nepean Blue Mountains Local Health District. We also extend our services to those who work or attend pre-school in the district.

To access our services, a referral through the Central Intake Service is required. During business hours, please call 1800 222 608 to initiate the referral process.
