To ensure accurate payment allocation, please provide the name and NHI (National Health Index) of the individual you are paying for in the designated spaces. As of 28 August 2020, all patients are automatically enrolled as members of NUHS and will remain so for a period of two years. However, starting from 28 August 2022, patients will need to apply for the new membership structure. We are excited to invite individuals who reside in the Central, Southern, and Eastern suburbs of Wellington and are 18 years of age or older to become members of NUHS. Membership applications will be carefully considered by the NUHS Board, and successful applicants will be notified accordingly.
