Welcome to Subiaco Adult Community Mental Health Clinic, located at Rokeby Road in Subiaco, Australia. As a State Government health service, our goal is to offer specialist mental health assessment and treatment to those living in the community.
At our clinic, we use the term "consumer" to describe the individuals who utilize our services. It is important to note that not everyone referred to us will require assistance from our Community Adult Mental Health Service. In such cases, we will provide information about other organizations that may better meet your needs. Additionally, upon your discharge from our service, you may return to the care of your general practitioner (GP).
Our community clinics are conveniently located in Butler, Wanneroo, Mirrabooka, Osborne Park, and Subiaco. These clinics are staffed by multidisciplinary teams consisting of medical staff, psychologists, nurses, occupational therapists, social workers, welfare officers, Aboriginal mental health workers, and peer support workers. They are further supported by General Practice Liaison Officers and Community Development Officers.
We offer a variety of services coordinated by our Community Treatment Teams, Assessment and Treatment Teams, Intensive Clinical Outreach Teams, NMHS MH Dialectical Behaviour Therapy Teams, Obstetric Liaison Consultation Service, and Post-natal Teams. Additionally, our Community Adult Mental Health includes the Neuromodulation Service and the Creative Expression Centre for Arts Therapy (CECAT), which utilizes creative mediums and processes to assist in mental illness recovery.
Referrals to our service can come from various sources, including family members, GPs, health professionals, other service providers, inpatient services, or even self-referral. Once you arrive at the North Metropolitan Community Adult Mental Health Service, you will be taken care of by a team of professionals who will work together to ensure you receive the best support and care.
Our team of clinicians includes psychiatrists, community mental health nurses, occupational therapists, social workers, and clinical psychologists. You may also receive support from a peer support worker, occupational therapy assistant, or Aboriginal mental health worker.
Every individual attending our Community Adult Mental Health Service is assigned a case manager who will oversee your care from entrance to discharge. Your case manager will coordinate recovery-oriented care, which involves assessment, treatment, and support for you, your carers, or significant others. Clear communication between all parties involved is a priority.
If you do not already have a GP, your case manager can assist you in finding one within the first few weeks of your care. This ensures that you will be familiar with your GP by the time you are discharged.
We also have peer support workers who have personal experience with mental health challenges. They are trained to support you in exploring your own recovery pathway. Additionally, Aboriginal mental health workers are available to provide support specifically for Aboriginal or Torres Strait Islander individuals.
Throughout your care at the North Metropolitan Community Adult Mental Health Service, your GP remains an important part of your team. Your psychiatrist will regularly communicate with your GP, keeping them up-to-date with your treatment. By the time you are discharged, your GP will already be aware of your care while with us.
All consumers in our Community Adult Mental Health Service complete an individual Care Plan. This plan is created collaboratively between you and your case manager and includes your wellness maintenance strategies, signs of distress, and recovery goals. You will have the opportunity to sign your Care Plan and receive a copy for your records.
If you have a carer or significant other who you would like to involve in your care, they may also be asked to participate in the development of your Care Plan. We believe in a holistic approach to mental health and recognize the importance of including your support network in your treatment.
Our mental health services adhere to national standards of care and legislation. The National Safety and Quality Health Service Standards (NSQHS) ensure a consistent level of care across health care organizations, while the National Standards in Mental Health Services provide a framework for safety and quality in mental health care. The Mental Health Act 2014 governs the provision of mental health care and treatment, outlining the rights of consumers and carers.
